I have rats and moles. I fill in the holes the next morning the holes are back. when i go to work at night I see the rats running back and forth

I have a lot of rats and mole in my yard. I have tried d-con, mole repellants and a lot of other stuff and they are still alive. the problem is getting worse. I am afraid the rats will eventually get in my house.
